If you are unable to access the form, please fill in the form on page 5 and 6 of the seminar information sheet and email the completed form to:

A confirmation email will be sent to the email address provided within 3 working days if you have secured your slot(s) for the seminar. Please note that places are limited and will be allocated on a first-come, first-served basis. Class is limited to 10 attendees.
Seminar registration fee is applicable at S$15 (before GST) per pax.
We will advise on the payment once the registration has been confirmed.
Registration will close 1 week before the seminar date.
